Viral Serological Markers and its Changes During Follow up in Acute and Chronic Liver Diseases

Abstract
|
|
|
The authors investigated viral serological markers and its changes follow up in 133 cases of acute and chronic liver diseases confirmed by liver needle biopsy. The results were as follows:
1) Positivities of HBsAg were 100% in acute viral hepatitis, 97.7% in chronic active hepatitis, 80% in primary liver cancer, 60% in liver cirrhosis, and 55.6 in chronic persistent hepatitis, respectively.
2) Anti-HBc was positive in majority of various liver disease (93.3 %) and appeared usually with HBsAg or anti-HBs, but single positive in minority of patients (5.1 %).
3) Positivities of HBeAg in HBsAg positive liver diseases were 87.8% in chronic active hapatitis, highestly. But positivity of HBeAg was no difference of chronic active hepatitis with or without cirrhosis
4) In 82 cases of HBsAg positive, seroconversion to anti-HBs was 2 cases (2.4 % ) in chronic active hepatitis and reactive hepatitis due to silicosis. And in 57 cases of HBeAg positive, seroconversion to anti-HBe was 4 cases (7. 0 f/0) in chronic active hepatitis.
5) By HBsAg positive was followed up more than 6 months in 82 cases of HBsAg positive, symptomatic chronic carrier was confirmed in 38 cases (46.3 %) . Of which, chronic active hepatitis was most common in 73.7%.
6) IgM-anti HBc of chronic carrier was negative in 23 cases of 24 cases(95.8%)
7) In chronic active hepatitis and chronic persistent hepatitis, ALT was more increased than AST (p <0. 05). Otherwise in primary liver cancer and alcoholic hepatitis, AST was more increased than ALT (p <0. 05). Also in chronic hepatitis, AST and ALT were more increased at HBeAg positive than anti-HBe positive.
On conclusion, most liver diseases in the Korean are associated with type B hepatitis and although HBeAg positive serum has been viral replication and infectivity, appearance of anti-HBe is no relationship of processing of liver cirrhosis.
Also, IgM-anti HBc by EIA may be help differenciation --cute and chronic liver diseases.
